The link below contains all of the forms (PDF format) new DLCC patients are required to fill out. Please click on the link below to download.

Please fill out the form, print it out and bring it with you on your first visit. Do not e-mail us the form. See instructions below.

You must have the free Adobe PDF Reader installed on your computer (most computers already do, but you can always get it here).

Instructions on how to fill out the form

1. Click on the download link above. You will get a choice between opening the file or saving it on your computer (if you're using Chrome, the file may be automatically saved).
2. If you decided to save the document, make sure you remember in which folder you saved the document. Open the PDF document you just saved by going to the folder where you saved it and double-clicking on the document.
3. Type in your information. You can click on each field to position your typing cursor there, or press the Tab key to advance to the next field (shift-Tab goes back to the previous field).
4. When you're done, print the document by clicking on the printer icon at the top of the document window, choosing "File" and then "Print..." on the menu.
5. When closing the document after printing, Acrobat will ask if you want to save the changes in the document before closing. Click the "No" button.
